Can I request a medical certificate for my insurance company over the phone?

For Japanese speakers
*If you need an interpreter during your consultation, please refer to the “For English (or other language) speakers”.

Applications can be made at the Medical Document Reception (Window 4) on the first floor.
Reception Hours: Monday to Friday, 9:00 to 17:00; Saturday, 9:00 to 13:00 (excluding Sundays and national holidays).

Please bring your identification and the designated form if there is a specific format required.
If someone other than the patient is making the request, a letter of authorization from the patient is also required.

How long does it take to get a medical certificate?

It typically takes about 1 month.

*Depending on the department and reception status, it may take more than a month.
*For certain types of documents, a follow-up visit may be required, or it may not be possible to issue the document.
