Admission Procedures
Required Documents for Admission:
- Admission Application and Pledge Form
*Please submit the Admission Application and Pledge Form to the Hospitalization and Discharge Information Counter (Window 7) on the 1st floor at Building A. - Hospital ID Card
- Health Insurance Card (including the Elderly Medical Insurance Certificate and Senior Recipient Certificate)
- Other Medical Care Eligibility Certificates (such as the Ceiling Amount Application Certificate)
- Personal Seal (Hanko)
Items to Bring for Hospital Admission
Please bring the following items when you are admitted to the hospital:
- Toiletries (razor, toothbrush)
- Tissues
- Underwear or diapers
- Indoor footwear
- Cup
- Earphones (If you are sharing a room with other patients, we kindly ask that you use earphones as a courtesy to them.)
- Medications and supplements which you are currently taking
- Medication handbook (“OKUSURI TECHO”) or documents that show the medications you are currently taking (if available)
*For infection control purpose, we kindly ask all visitors to wear a mask.
*For childbirth or surgery, specific instructions will be provided at the respective outpatient departments.
*Chopsticks and spoons are provided with the meals.
*We offer rental services for sleepwear, bath towels, and face towels.
*Starting from April 1, 2017, we have introduced a hospital admission set (CS Set) for new admissions. If you prefer to use your own items, please inform us.
Handling of Cash and Valuables
- For security reasons, please refrain from bringing large amounts of cash when you are admitted to the hospital, as we cannot take responsibility for its safekeeping. (Hospital bills can be paid by credit card, and there is a cash dispenser available at the Lawson convenience store in the hospital.)
- We cannot be held responsible for the loss of cash or valuables (such as glasses, hearing aids, dentures, and other prosthetic devices, as well as jewelry like necklaces and rings). Patients and their families are responsible for managing these items during the hospital stay. Particularly for dentures, there are many situations during treatment (such as examinations and surgeries) where they will not be worn. Proper storage and cleaning of dentures are essential. If you use dentures, please bring a “denture case” and “denture cleaner” with you when you are admitted.
On the day of admission
- Please arrive at the General Information Counter at Window 1 on the 1st floor of Building A by the designated time. Our staff will inform the International Medical Support Center of your arrival.
- Accompanied by a staff of the International Medical Support Center, proceed to the Hospitalization and Discharge Information Counter (Window 7) at Building A to check in for your hospital stay.
- Receive the necessary documents and the staff of the International Medical Support Center will escort you to the ward.

*The cooler can be used with a prepaid card, but it is not a refrigerator.
*Charges are per day, not per night (one night and two days). If you stay past midnight, you will be charged for two days.
*For rooms other than special private rooms, a TV card is required to use the TV.
Hospitalization Fees & Payment
Hospitalization Fee
- We do not charge any advance payments or referral fees.
- In Japan’s national health insurance system, the cost of medical services, including consultations, prescriptions, hospitalization, surgeries, and more, is calculated with a unit price per medical service fee point.
- Costs for childbirth, other self-financed treatments, private room charges, document fees, etc., will be charged according to the hospital’s prescribed rates.

Billing and Payment
- Upon discharge, your costs will be calculated, and you will be provided with a billing statement. Please make the payment at the Hospitalization and Discharge Information Counter (Window 7) & Accounting Counter (Window 5) on the 1st floor at Building A.
- For hospitalization charges, the fees for each month from the 1st to the last day will be calculated, and an invoice will be provided to you or your family after the 10th of the following month. Please make the payment at the Hospitalization and Discharge Information Counter (Window 7) & Accounting Counter (Window 5) at Building A.
*Our hospital has adopted the Diagnosis Procedure Combination (DPC) payment system since July 2006.

For Visitors
At our hospital, we will provide the following guidance regarding visits to inpatients starting December 29, 2024.
Visitation Restrictions (from December 29, 2024)
Important Notes
- We prioritize the healing and well-being of our patients, and we ask for your understanding and cooperation.
- These guidelines may change based on the patient’s condition and the status of infection outbreaks.
- For more details, please refer to the “Hospital Admission Guide” pamphlet.
We also request that all visitors wear masks.
Visitation Registration and Hours
Please register at the designated reception area within the visitation hours. Fill out the visitation application form (visitation card) available at the registration desk and submit it to the reception.
Visitation Guideline
- Please adhere to the designated visitation hours to avoid disturbing patients’ rest and medical care.
- Visitation may be restricted based on the patient’s condition. We appreciate your understanding.
- Avoid visiting in large groups as it can disturb not only the patient you are visiting but also other patients.
- Eating and drinking are not allowed during visits in the wards.
- Visitors under the influence of alcohol are not permitted.
- Our premises are entirely non-smoking.
- Information about the patient’s condition is only shared with family members or specific individuals. We do not provide updates over the phone.
- If you are unsure of the patient’s room location, please inquire at the Admission and Discharge Information Counter and Accounting Counter on the first floor.
To Patients, Families, and Visitors
To minimize the risk of infection, we prohibit the bringing of fresh flowers, potted plants, and similar items into the hospital from an infection control perspective. Thank you for your understanding and cooperation.
